CYP3A4 a member of the cytochrome P450 monooxygenase family is a membrane-bound protein which oxidatively metabolizes a wide range of organic compounds, including over 50% of all drugs currently prescribed. Predominantly expressed in the liver and small intestine, it is the most abundant of the P450’s in humans. Like CYP2C19, it shows substantial genetic polymorphism, leading to wide inter-individual variability in rates of drug metabolism. Common inhibitors are ketoconazole, cimetidine and grapefruit juice. Endogenous chemicals such as arachidonic acid are also substrates, forming EET’s and 20-HETE with a variety of physiologic effects.
