Thrombin is a key factor in the coagulation pathway. Upon thrombin stimulation, platelets accelerate oxidative phosphorylation to a high level to support the energy intensive aggregation and clot formation process. This process utilizes glutamine and fatty acids rather than glucose. Beyond the well-known participation of thrombin in the clotting process, there are other areas of metabolism in which thrombin participates. In mesangial cells, thrombin increases hexokinase activity and glucose metabolism. In neurons, thrombin modulates calcium homeostasis which in turn is linked to energy production and cell metabolism. In conditions such as type II diabetes, the combination of hyperglycemia and hyperinsulinemia contributes to a procoagulant state, partly mediated by increased thrombin generation. This can lead to a vicious cycle, where altered metabolism promotes thrombin generation, which in turn affects cellular metabolism.
