DESCRIPTION

Phosphoenolpyruvate (PEP) is a high‑energy glycolytic intermediate at a major branch point between energy production and anabolic carbon export. Its concentration and flux are critical for metabolic homeostasis. Enzyme dysregulation in generation or consumption of PEP are linked to disorders of glucose homeostasis, inborn errors of metabolism, and to conditions such as type 2 diabetes. PEP is generated from 2‑phospho-glycerate by enolase and converted to pyruvate by pyruvate kinase, driving substrate‑level phosphorylation of ADP to ATP in the final step of glycolysis. In gluconeogenic tissues, PEP is formed from oxaloacetate by phosphoenolpyruvate carboxykinase linking mitochondrial TCA-cycle intermediates to cytosolic glucose production. Excessive hepatic PEP production via overexpressed PEPCK contributes to fasting hyperglycemia in type 2 diabetes and metabolic syndrome by increasing endogenous glucose output.
